This is brought to your notice fraudulent activity happens with me. I am an account holder with the Punjab National Bank, Fountain Chowk branch Gurgaon. Some unknown person has withdrawn *****/- rupee against Cheque No-****** on dated **.**.****, while this cheque no is already intact in my cheque book.
